Requests for Verification of Attendance and Graduation:
The Office of Academic and Student Affairs processes requests for state board applications, military commissions, employer credentialing, or related requests. Please submit the specific request and corresponding paperwork to Rene Maldonado by e-mail at a.rene@uky.edu, fax 859-257-7297, or by mail at 789 S. Limestone St., Lexington, KY 40536.
Official Transcript requests for College of Pharmacy Graduates are processed through the Office of the Registrar. Please note there may be a delay for graduates prior to 1988.
Poster Printing Request
The College's ITS Department provides poster printing services exclusively for UKCOP faculty, staff, and students. This service is not available for other campus units or personal printing requests.
As a recharge operation, our goal is to offer high-quality printing at a significantly reduced cost—typically less than half the price of external vendors—rather than generating profit. The recharge rate is reviewed and set annually by the Assistant Dean for Finance and Administration to ensure continued affordability.
