Reporting Surveys

For the fiscal year that closes June 31, report data is collected July 1 through Sept 1. Please submit your domain report as soon as possible.

Faculty Expertise Update Form

Submissions are used to update the UKCOP website annually. Only the information you designated will also be used as a reference for media inquiries. If you're missing from the "Name" dropdown, please email kristie.colon@uky.edu.

 

Staff Data Report

You will automatically be prompted to submit another form once complete. Please submit a form for each unique data set, including Full-Time and Part-Time staff.

 

Preceptor Data Report

Data should be entered by the Office of External Studies to capture the number of voluntary faculty for the current fiscal year.

PPS Faculty Awards Report

This data should be submitted by the Department Chair or the Executive Assistance for the department. If a faculty member is not listed in the survey selection, please contact kristie.colon@uky.edu.

Gifts & Scholarships Report

Please report on the total number of gifts given for the current fiscal year.

 

We wish to remember and honor those who inhabited this Commonwealth before the arrival of the Europeans. Briefly occupying these lands were the Osage, Wyndott tribe, and Miami peoples. The Adena and Hopewell peoples, who are recognized by the naming of the time period in which they resided here, were here more permanently. Some of their mounds remain in the Lexington area, including at UK’s Adena Park.

In more recent years, the Cherokee occupied southeast Kentucky, the Yuchi southwest Kentucky, the Chickasaw extreme western Kentucky and the Shawnee central Kentucky including what is now the city of Lexington. The Shawnee left when colonization pushed through the Appalachian Mountains. Lower Shawnee Town ceremonial grounds are still visible in Greenup County.

We honor the first inhabitants who were here, respect their culture, and acknowledge the presence of their descendants who are here today in all walks of life including fellow pharmacists and healthcare professionals.
